What Is Androgenetic Alopecia?
Female androgenetic alopecia (FAGA), also known as female pattern baldness (FPB), is the most prevalent form of hair loss among women. It is a chronic genetic and hormonal condition that can occur at any age after puberty, and its frequency increases with age.
Regardless of the term used, androgenetic alopecia implies that you have inherited genes that cause your hair follicles to shrink and eventually stop growing hair. This shrinking process may begin as early as your teenage years, but it usually starts later in life and can be aggravated by hormonal changes during motherhood, perimenopause, and menopause.

What Causes Androgenetic Alopecia?
Female androgenetic alopecia is a condition that is caused by a combination of genetic and hormonal factors. The adrenal glands and ovaries naturally secrete male hormones known as androgens, and if there is a genetic predisposition to androgen sensitivity, it can lead to androgenetic alopecia. This increased sensitivity of the hair roots to the secretion of male hormones, due to genetic predisposition, results in a faster hair growth cycle, which in turn leads to the early exhaustion of hair follicles.

What Are the Stages of Androgenetic Alopecia?
Androgenetic alopecia affects women differently than male pattern baldness. Women with this condition experience hair loss in various parts of their scalp and body, unlike men who usually experience it at the crown of their head. The first noticeable sign of androgenetic hair loss in women is usually overall thinning or a widening of the parting.
According to the Sinclair scale, androgenetic alopecia progresses through five stages:
Stage 1: Little to no hair loss.
Stage 2: A slight gap appears in your center hair parting
Stage 3: A wider gap is present in your center hair parting, and there is hair loss on both sides of your parting line.
Stage 4: Bald spots appear towards the front of your hairline.
Stage 5: Advanced hair loss.
Can You Reverse Androgenetic Alopecia?
If you’ve lost hair from androgenetic alopecia, it won’t grow back without intervention. Proper treatment can stop hair loss and help regrow hair.

How Common Is Androgenetic Alopecia?
Androgenetic alopecia also know as female pattern baldness, affects approximately 30 million women in the United States and is a condition that is becoming more common.
What Medical Treatments Are Available?
It is important to take your time and research the available medical treatments thoroughly, as all treatments come with published side effects. Ultimately, the decision of which treatment to choose is a personal one.
Topical therapies are often used as a first-line treatment for androgenetic alopecia, particularly during early hair loss. One such therapy includes using topical minoxidil, which is commonly known by the brand name Rogaine.
According to the American Academy of Dermatology (AAD), minoxidil works by stimulating hair growth and preventing new hair loss. However, it may take consistent use for three to six months before any benefits are seen.
One drawback of using minoxidil is that you need to use it daily and as directed. Failure to do so may cause thinning of hair even after it has grown back.
Another option is finasteride, also sold under the brand name Propecia, which has been approved by the FDA. However, this medication has been linked to causing birth defects, so it should only be used by postmenopausal women. In such cases, Propecia is offered as an off-label prescription, similar to how oral minoxidil is prescribed for hair loss in general.
It's important to note that this medication is not suitable for women who are pregnant or may become pregnant due to the potential risk of birth defects.
Laser Treatment
Low-level laser therapy (LLLT) stimulates hair follicles and prolongs their growth phase, resulting in thicker hair and greater scalp coverage. It's a safe and effective treatment for hair loss in both men and women, especially when done for more than 16 weeks. LLLT devices, such as helmets or caps, are available for purchase and use at home. Consult your dermatologist for the best options and consider combining LLLT with other treatments for better results.
Note that LLLT devices can be expensive and challenging for some women to fit usage into daily life.
Microneedling
Microneedling on the scalp can stimulate hair growth by puncturing the skin with tiny needles that trigger inflammation, which stimulates collagen production. Research shows that using microneedling alongside topical minoxidil can improve hair growth and thickness. It can also be used with platelet-rich plasma therapy.
Microneedling can be a painful procedure with varying results.
Platelet-Rich Plasma Therapy
Platelet-rich plasma therapy (PRP) is a procedure where a dermatologist injects plasma from your blood into the areas of your scalp where hair is thinning. This is done in a series of procedures and requires regular maintenance to slow hair loss, according to the American Academy of Dermatology (AAD). Although not approved by the FDA, PRP is used off-label and may work by increasing blood flow to hair follicles and reducing inflammation.
While PRP has shown some promise, the available data to support its use is limited.
Surgical Hair Transplant
Hair transplant surgery involves taking hair from the back of the head and moving it to the top. It's expensive, costing thousands or even tens of thousands of dollars, and not covered by health insurance. It's recommended for people who have lost 50-75% of the hair on top of their head, or all of it.
Recovery can take months, and multiple surgeries may be needed for the desired result.
